ACL Natural Healing Case Report #52: Soccer Injury Restored to ACLOAS Grade 1 via Strict Consensus Rule (27-Year-Old Italian Male)
Summary:
The patient sustained a right knee ACL rupture during a soccer turn. Based on a strict consensus rule among three independent specialists, the initial diagnosis was Ihara Type III (tear with displaced stumps). He initiated Evo-Devo Exercise via remote online guidance. He strictly followed a rigorous environment: a walking limit of ≤3,000 steps per day and a 30–120° brace ROM restriction. After 4 months, the final MRI evaluation showed a remarkable result. Patient Information
- Age / Nationality: 27-year-old male, Italian
- Injured side: Right knee
Mechanism of Injury
- Date of injury: 17 April 2023
- Context: Injured the right knee while turning left during a soccer match. The knee twisted inward. Initially, he underwent conventional physical therapy (PT) for quadriceps strengthening. However, he later chose natural healing and consulted our clinic.

Initial Evaluation (MRI #1)
- Date: 12 May 2023
- Diagnosis: Right knee complete ACL rupture
- Ihara classification: III (Grade 3) — Confirmed by consensus of 3 independent specialists (tear with displaced stumps)
- Note: The individual diagnoses were Ihara Grade 2, 3, and 3. Therefore, the final consensus was determined as Grade 3.

Initial Policy & Start of Therapy
- Start date: 22 May 2023
- Format: Online guidance; initiation of Naturalization Therapy (Evo-Devo Exercise)
- Brace management: Knee ROM strictly restricted to 30–120°
- Activity: Walking strictly restricted to ≤3,000 steps/day
- Home program: Evo-Devo Exercise 3 times/day for 4 months


Final MRI Evaluation (~4 months)
- Date: 21 September 2023
- Assessment: ACLOAS Grade 1 (good morphology and continuity) — Unanimously confirmed by 3 specialists (1, 1, 1)
- Finding: High-quality natural regeneration was achieved. The ligament shows clear continuity and excellent stability.

Author’s Observations
- First, achieving “ACLOAS Grade 1” from an initial “Ihara Type III” is a remarkable outcome. All three specialists agreed on the final high-quality result.
- Second, the patient’s discipline in following the ≤3,000 step limit was a major factor. For an active 27-year-old soccer player, this patience led to biological success.
- Third, switching from conventional PT to Evo-Devo Exercise was crucial. This change allowed the ligament to heal without being disrupted by premature exercise.
